Introduction
"Botteghe di Comunità" is a management experiment of Community Medicine by ASL Salerno (local health company) to respond to the health and social needs of the internal areas of Cilento (district n° 69 and 70), that are affected by logistical problems due to the distance from accident and emergency points or hospitals, or due to the presence of difficult mountain routes to cross, and where there is a high percentage of elderly population.


Purpose
The aim of "Botteghe di Comunità" is to promote the health through the continuity of care services, supporting the activities of primary care in 29 locations of the internal areas, named "spoke". Their centrality is given by the functions of initiative health: taking charge, unified access, filtering access, and directing patients.


Method
"Botteghe di Comunità" HUB are local socio-healthcare centres where a nurse is assisted by specialists and other local professionals, and by civil service volunteers and city competence. The Botteghe also host General Practitioners and the Territorial Functional Aggregations - AFT of the area. The service is provided 5 days a week, and each day is dedicated to a specific branch, also through telemedicine, for the management of chronic diseases with higher prevalence, i.e. diabetes, cardiovascular diseases, chronic respiratory diseases, cerebrovascular pathologies including dementia and poly-pathologies of the frail elderly.


Results
With "Botteghe di Comunità", ASL Salerno spreads Community Medicine around the territory, specifically in the internal areas, through an healthcare model for managing chronic diseases based on proactive assistance to the individual from the stages of prevention and health education to the early and established phases of the illness.


Conclusions
"Botteghe di Comunità" experiment increases quality of life of people who live in the internal areas and improves the manage of chronic diseases to reduce the inequalities of health care.
