HEALTH-RELATED QUALITY OF LIFE AFTER PARATHYROIDECTOMY IN PRIMARY HYPERPARATHYROIDISM USING SF36 AND PASIEKA'S PARATHYROID SYMPTOMS SCORE

Background:
Primary hyperparathyroidism (PHPT)is the 3rd most common endocrine disorder. Musculoskeletal involvement is reported in about 53 % of patients, which impairs the quality of life (QoL). As the objective data addressing the QoL of these patients from India is limited; therefore, the purpose of the current study was to evaluate the QoL using SF 36 and Pasieka's symptoms score before and after curative parathyroidectomy (PTX) in patients with symptomatic PHPT.
Methods:
This was a prospective observational study which included 31 symptomatic PHPT patients who underwent curative PTX. QoL was evaluated before and three months after PTX using SF-36 questionnaire and Pasieka's Symptoms score.
Results:
Mean age of patients was 41.65 ±14.71 years with involvement of musculoskeletal system in 83.87 %. Successful parathyroidectomy was followed by a decrease in severity of symptoms and improvement in QoL both on SF36 and on Pasieka symptom score (p<0.001). A positive correlation was found between hypercalcemia and all components of Pasieka scale. However, positive correlation was found only preoperatively between mild hypercalcemia and Mental health components on SF36.
Conclusions:
The results of this study showed that the disease specific Pasieka symptoms score has good psychometric properties, which makes it valid analysis tool for the evaluation of patients with PHPT in clinical practice. Hypercalcemia of all grades have significant effect on symptomatology in Pasieka symptoms score as compared to SF 36.
